To sign up for direct deposit:

  • Fill out the Direct Deposit Application Form and mail it to the address found on the top left hand corner of the application.
  • If you are depositing into a checking account, you will need to include a voided check (not a deposit slip) along with the application form.
  • If you are depositing into a savings account, you will need to include a letter prepared by your financial institution on its letterhead, along with the direct deposit application. The letter must include your savings account number and the financial institution's routing number.
  • If you do not have a printer, you can request a mailed copy of the direct deposit application form by calling the KIDS Info Line at 1-800-991-5530.

To change your direct deposit account:

  • Fill out a new direct deposit application form.
  • In the middle section of the form, check the box that states "I want to - Change My Account".
  • Ensure your new account information is entered correctly.
  • Mail your form to the address found on the top left hand corner of the application.

To end direct deposit and receive money on the EPPIC Debit MasterCard:

  • You may fill out a direct deposit application form, choosing "I want to - Cancel Direct Deposit".
  • You may call the KIDS Info Line at 1-800-991-5530 and request cancellation of your direct deposit.
  • Your next support payment will automatically enroll you in the debit card program and you will receive any future payments on an EPPIC Debit MasterCard.
