Listed below are reports of illegally operated, revoked, and denied child care providers in the State of Wisconsin. The reports are updated weekly, and they are in portable document (pdf) format. To view documents in pdf format, you need Adobe Acrobat Reader. Questions about these reports should be directed to DCFCCLICREG@wisconsin.gov.

  • Illegally Operated Wisconsin Child Care Providers: This report lists all illegally operated child care providers (caring for 4 or more children under age 7 who are not related to the child care provider) who have been issued a stop operating order by the Department of Children and Families within the past 5 years.
  • Revoked Wisconsin Child Care Providers: This report lists all child care providers who have had their license or certification to operate a child care center revoked by the Department of Children and Families or certification revoked by the certification agency within the past 5 years.
  • Denied Providers: This report lists all child care providers who have been denied a license or certificate to operate a child care center by the Department of Children and Families, or denied a certificate by the certification agency within the past 5 years.
