If you have a concern about something going on in a child care facility or you suspect child abuse, neglect or exploitation at a child care facility, please use the drop-down menu on the Tell Us How We Are Doing page to fill out the appropriate complaint form.

If you run out of a particular YoungStar brochure, you can order more from the Department of Administration (DOA) Document Sales & Distribution Center.

  2. Under the "Browse By Agency" section at the top of the page, click on "Department of Children and Families" to view the complete list of forms and publications that are available to order. Be aware that many items are packaged in multiple quantities (e.g., Pkg. of 25).
