Foster Parent Champion Reward Request

The Department of Children and Families wants to extend appreciation to foster parents who successfully recruit new foster parents throughout the state by providing a reward for being a Foster Parent Champion. To be eligible for this reward, the recruited foster parent(s) has to be licensed and in their first licensing period (i.e. not previously licensed by another licensing agency or at a different point in time).

This webform replaces the former process for licensing agencies to submit referrals for Foster Parent Champions. Please enter the information below so your Foster Parent Champion reward request will be processed. These requests are reviewed quarterly the following months: March, June, September, and December. Once DCF verifies licensure of the recruited foster parent(s), an electronic $100 Visa giftcard will be emailed to the foster parent champion.
