Child Welfare CQI Reports

In 2014, DCF remodeled its Continuous Quality Improvement (CQI) system. The CQI process identifies areas of strength and areas needing improvement using many sources of data. This includes information gathered from case record reviews.

After the initial round of case record reviews were completed in the areas of Access, Initial Assessment, and Ongoing Services, reports were published that provide information on each process. This includes the background, methodology, training and quality assurance of the reviewers, key findings, and next steps. After publication of the initial reports in 2015, DCF developed Access and Initial Assessment dashboards that illustrated key outcomes from these review processes.

Since 2015, Ongoing case reviews have continued to be conducted as part of the Child and Family Services Review (CFSR) process. DCF reports results from the Ongoing Services reviews to the Children’s Bureau quarterly as required in the Program Improvement Monitoring Plan.

Access Case Record Reviews

Archive - 2015 Access Report Information

In 2015, an in-depth report captured the framework of the Access review process. The report is comprised of three sections:

  • The Executive Summary offers overall key findings and recommendations of the Access Case Reviews and discusses the procedural lessons that were learned.
  • The 2015 Access Case Record Review Report is an in-depth report that outlines the information included in the Access Case Reviews.
  • The 2015 Access Case Record Review Appendices includes additional information that supplements the 2015 Access Case Record Review Report, including a copy of the Access Case Record Review Instrument.

After 2015, the creation of 1-page annual reports highlight the results of the Access reviews conducted.

  • This Report provides a summary of results related to Access, Initial Assessment, and Ongoing Services Case Record Reviews completed during the years 2015 and 2016 that pertain to Indian Children and practices required under the Wisconsin Indian Child Welfare Act (WICWA).
