Assessment and Stabilization Centers Milwaukee County Child Protective Services Information for Agencies licensed through DCF 59 DMCPS requires that shelter care facilities operating as Assessment and Stabilization Centers (ASCs) for DMCPS and used as placement options for children in DMCPS care must be under contract with DMCPS. Shelter care facilities must apply and submit the required Application with budget no later than the date outline on the provided Application form. Links to the Application form and the Scope of Services for the next contract year are below. Note: The Application deadline has been extended until Monday, November 6th, 2023 at 4:59pm. Assessment / Stabilization Center (DCF 59) Application Form Updated Annual Budget required to be submitted with application 2024 Scope of Service Be sure to submit a completed Application form to avoid an application not being accepted. The information in your application form will also be used by the Contracted Case Management Agencies and the Placement Referral Unit to make appropriate placement referrals for children/youth. Following a satisfactory paper review, a site visit may be scheduled by the DMCPS Contract Administrator. Please note: Although a contract may be granted, DMCPS does not guarantee the placement of children within any ASC. All placements are determined based on the specific needs of the child.
