Paper Application Process for Recertification – Milwaukee County Certification Your family child care certification must be renewed every two (2) years. You will need to submit the following at least two (2) months prior to the expiration of your certification to avoid any lapses in certification: Completed and signed recertification application Complete, sign, and submit the recertification application. $90.75 nonrefundable recertification fee The total cost for a two (2) year certification is $90.75, and it is nonrefundable. Submit payment via check, cashier’s check, or money order made payable to the Department of Children and Families, or through the e-payment portal in the Child Care Provider Portal using a credit card, debit card, or ACH bank transfer. Access the e-payment module through the Child Care Provider Portal. Unpaid fees will delay the recertification process. Any past due fees must be paid before recertification can be approved. Training Documentation Certified child care operators and all employees and substitutes working at your program must complete five (5) hours of qualifying continuing education per year. Submit documentation of all continuing education completed during the previous 2 years by the operator, employees, and substitutes. Time spent renewing infant/child cardiopulmonary resuscitation (CPR) training may be counted toward the required continuing education hours. Background Check Request (BCR) form All persons 10 years of age or older living or working in the home where child care is being provided must have a BCR form on file. For minors 10 through 17 years of age, the BCR must be completed and signed by the parent/guardian. If there have been changes to your household members since the last certification application: Contact your certifier. Submit a completed Background Check Request (BCR) form for any new person 10 years of age or older who is living or working in the home. Processing your recertification could take up to 60 days. Once your paperwork has been processed, a certification specialist will conduct a home visit.
