Adding or editing a barrier The following document focuses on adding a new barrier or editing an existing barrier. Below you can find additional information on: Barrier Details The “Barrier Details” section is used to record general information about a barrier. It contains the following questions: Unlike the informal assessment, you will need to complete all required fields before you can save a barrier. If the participant already has a treatment provider, you can add their contact information in this section. Be sure to select the “Treatment/Assessment Provider” contact title. You can even add multiple contacts, if needed. Formal Assessment The “Formal Assessment” section is used to enter information about formal assessments. It contains the following fields: You will notice that there are links to forms in this section, which are primarily used for the W-2 program. Once you add an “Assessment Date”, additional fields are displayed / required, as shown below: If the participant has had multiple formal assessments, you can click the “Add Formal Assessment” button to add additional formal assessments: You can add up to three formal assessments at a time per barrier. To delete a formal assessment, click the trash can icon: You will need to select a “Delete Reason” when deleting a formal assessment. Accommodations Once an “Assessment Date” has been entered, you will be required to select “Yes” or “No” to the following question: If you select "Yes", you will have the ability to add additional details about the accommodation(s): Keeping Protected Information Safe The “Participant Barriers app” includes an extra layer of security, so it’s okay to enter personal health information (PHI) about the participant on this page. Personal health information can include details about medical conditions that may affect the participant, if relevant for case management. The “Participant Barriers app” is not available to TMJ/TJ workers Remember, the “Participant Barriers app” is one of only a handful of places in WWP where it’s okay to enter personal health information. The others include the “Family Barriers” page, the “Participant Barriers” page, and contacts with the “Treatment/Assessment Provider” title. All of these pages include the extra security needed to protect sensitive information about the participant and their family. Help for Individuals with Read-only Access If you have read-only access to WWP, you will able to see the response selected for each question. However, in cases where the question included a list of options, it may be helpful to see the full list of choices.
