Family Foundations Home Visiting Program - Statewide

Family Foundations Home Visiting Program uses evidence-based home visiting models to provide a voluntary program offering supportive services to parents and their children during pregnancy and throughout early childhood years. Direct home visits address but not limited to: 

  1. Prenatal care access 
  2. Screenings & Assessments of parent and child health and well-being 
  3. Health education 
  4. Parent-child relationships 
  5. Child development/age-appropriate expectations 
  6. Connections to community resources

Private, public agencies and tribes are eligible to apply either individually or in partnership with others in the same service area. It is DCF’s intent to award contracts across all regions of WI, with an anticipated contract start date of October 1, 2027.
