Statutorily Required Information for Child Placing Agencies

This form collects foster home licensing information for child placing agencies without access to eWiSACWIS, as required under Wis. Stat. § 48.60 and Wis. Admin. Code ch. DCF 56. A separate form is required for each licensing action and foster home.

Please do not submit duplicate requests. A response will be provided for initial licenses within 10 business days of receipt. If you have not been contacted after 10 business days, you may send an inquiry-only email to DCFCPALicensingRequests@wisconsin.gov, including the foster home’s name.
