This page gathers basic information about the applicant.  

Application

The Application Date is the date the individual completed and signed the EA application. The Application Initiated Method is required. Use the drop down to indicate how the meeting was held.

Residency, Address, and Contact Information

The County of Residence and Address are required in the first portion of this section. You must verify that the applicant is a Wisconsin resident. Use the drop-down menu to select the verification provided for WI Residency Verification. If the applicant is homeless, you must enter a mailing address. 

You must Validate the applicant’s address. This validates the address against the United States Postal Service (USPS) database. The Validate button turns blue and is clickable after you enter address information. After clicking Validate, the Address Search Results display several options:

  • Use the address suggested – The USPS returned a result for the address entered;
  • Resubmit the address – If the address was entered incorrectly; or
  • Use the address entered – If the address is correct but says Invalid from the USPS results.

Select the appropriate option based on the search of the applicant’s address you entered. 

Enter the applicant’s phone number, how best to reach them, and email address if applicable. 

Caretaker Questions

These questions are required. If the applicant answers No to these questions, they are ineligible for EA because there needs to be a Caretaker Relative of at least one dependent child. If this is the case, click Save +Continue, then move to the Agency Summary page to deny the application.  

If there is a Caretaker Relative of at least on dependent child, continue with the application driver flow after clicking Save +Continue.
