Request for Assistance The “Request for Assistance” page allows you to add a request for assistance for TMJ, TJ, CF, or ELEVATE, including an eligibility determination for TMJ and TJ. It allows you to view current and past requests for assistance. Below you can find additional information on: Accessing the Requests for Assistance page You can access the “Requests for Assistance” page at any time from the “Case Management” menu when a participant is selected, as shown below: However, in order to add a new request for assistance, you first want to complete “Clearance” and “Client Registration” for the participant. Adding a Request for Assistance – CF To add a “Request for Assistance” for CF, click the “Add Request for Assistance” button: Select “Children First” as the “Program”. “County of Residence” will be auto-populated with the value selected on the “Client Registration” page. Once you have selected “Children First” as the program, you will see several other field appear on the page: Fill out the “County/Tribe Where Court Ordered”, “Effective Date of Court Order”, and “WP Office”. Once these field have been completed, you will see that the “Refer” button is enabled in the bottom right-hand corner of the screen. Once you click the “Refer” button, the participant will be in “Referred” status for CF. Adding a Request for Assistance – TMJ/TJ To add a “Request for Assistance” for TMJ or TJ, click the “Add Request for Assistance” button: Select “Transform Milwaukee Jobs” or “Transitional Jobs” as the “Program”. “County of Residence” will be auto-populated with the value selected on the “Client Registration” page. Once you select “Transform Milwaukee Jobs” or “Transitional Jobs”, additional fields will display. You will be required to enter the following information: Contractor – select the company you work for from the dropdown menu Application Date Is the applicant the biological/adoptive parent – or relative and primary caregiver – of child(ren) under the age of 18? Household Size Annual Household Income Last Date of Employment – you can select “Never Employed” if the applicant has never been employed Eligible for Unemployment Insurance Benefits Is applicant receiving W-2 benefits or services? Is applicant a U.S. citizen or qualified non-citizen? Has applicant worked a total of 1,040 hours in TMJ/TJ in the past? Has application been completed and signed? Has eligibility information been verified? Is applicant able to obtain and benefit from a subsidized job? Several of these questions will prompt follow up questions, depending on the response you select. If you select “Yes” to “Is the applicant the biological/adoptive parent – or relative and primary caregiver – of child(ren) under the age of 18?”, you will be required to enter the following information for each child: First Name Last Name DOB Gender For “Last Date of Employment”, if you enter a date within the last four weeks, you will have to answer the following question: In the last four weeks, has the applicant worked an average of 16 or less hours per week? If you select “No” for “Is applicant able to obtain and benefit from a subsidized job?”, you are required to enter details. After you enter all the information outlined above, you will need to hit the “Determine Eligibility” button. At this point, if you have made any errors – for example, leaving a required field blank or typing a date in the wrong format – WWP will display an error message at the top of the page. You will need to correct any errors listed there, then hit the “Determine Eligibility” button again. If there are no errors, WWP will either display denial reason(s) or indicate that the participant has passed eligibility. Regardless of the outcome, you will need to select a population type. The following are potential options: F1 – In out-of-home care or transitioning to independent living G0 – Adult youth with no children G1 – Parent with a child support order G2 – Parent under a reunification plan G3 – Parent who is an ex-offender Does not meet criteria for target population Some may be excluded or greyed out, depending on your contract agency, program, and applicant characteristics. After selecting a population type, you will have the ability to select either “Refer” or “Deny”. If you select “Refer”, the participant will be set to “Referred” status for your program. You will also have the option to “Save” which allows you to keep the RFA “In Progress”. Adding a Request for Assistance – ELEVATE To add a “Request for Assistance” for ELEVATE, click the “Add Request for Assistance” button: Select “ELEVATE” as the “Program”. “County of Residence” will be auto-populated with the value selected on the “Client Registration” page. Once you have selected "ELEVATE" as the program, you will see several other fields appears on the page: Fill out the "KIDS PIN", "Referral Source", "Court Ordered or Voluntary?" and "County of Service". Once these fields have been completed, you will see that the "Refer" button is enabled in the bottom right-hand corner of the screen. Once you click the "Refer" button, the participant will be in "Referred" status for ELEVATE. Viewing Requests for Assistance If the participant has existing TMJ, TJ, CF, or ELEVATE RFA(s), you can view them on the “Requests for Assistance” page: By default, only RFAs created in WWP will be displayed. If you want to see RFAs created in CARES mainframe, click the “Display CARES mainframe RFAs” button: ELEVATE will not have "Display CARES mainframe RFAs" button.
