My Complaint is about an agency decision.

You have the right to ask for a Fact Finding review if you think that the W-2 agency made a wrong decision about your Job Access Loan, Emergency Assistance, W-2 application, placement, or payment.

A Fact Finding review is a meeting where you and your W-2 agency both give information to the Fact Finder. The Fact Finder will listen to both sides, and then decide if the W-2 agency made a wrong decision. If the Fact Finder decides that the W-2 agency made a wrong decision, the Fact Finder may order the W-2 agency to change its decision.

You must ask for the Fact Finding review within 45 days from whichever of these is later:

  • the date of the notice you disagree with; or
  • the date the decision announced in your notice takes effect.

You may ask for a Fact Finding if you think the W-2 agency:

  • was wrong in its decision to deny your W-2 application;
  • took too long to start your W-2;
  • put you in the wrong W-2 placement; or
  • wrongfully reduced or ended your W-2 payments.

Contacting the Department of Children and Families on this web site is not the same as requesting a Fact Finding. To request a Fact Finding, you must contact the W-2 agency that made the decision you think is wrong.

To request a Fact Finding, you may do any of these:

  • go to your W-2 agency and tell them you'd like to have a Fact Finding;
  • call your agency and tell them you'd like to have a Fact Finding;
  • fill-out the Fact Finding Request form and mail, email, fax, or bring it to your W-2 agency.

Your W-2 agency will notify you of your scheduled Fact Finding Review appointment within 3 working days after they receive your request.
