Care4Kids

The Department of Children and Families (DCF) and the Department of Health Services (DHS) are partnering to put a new program in place improving the health of children in out-of-home care.

Care4Kids is a new Medicaid program following a medical home model for children in out-of-home care started on January 1, 2014.  Care4Kids is a pilot program in six counties in the southeastern region of Wisconsin. 

 

Who is eligible for Care4Kids?

Children placed in out-of-home care in the following counties may be eligible for Care4Kids:

  • Milwaukee County
  • Kenosha County
  • Ozaukee County
  • Racine County
  • Washington County
  • Waukesha County

What is the goal of Care4Kids?

The goal of Care4Kids is to improve healthcare for children in out-of-home care by:

  • Strengthening the quality of care
  • Increasing access to care
  • Improving timeliness of care
  • Improving physical and mental health for children
  • Promoting improved resiliency
  • Creating stronger natural supports

What do children enrolled in Care4Kids receive?

Children in Care4Kids have a team of professionals who:

  • Help coordinate health care services
  • Help get comprehensive health care coverage
  • Assign primary care physicians who have training on the needs of children in out-of-home care
  • Develop a health care plan for the child

Children in Care4Kids also receive:

  • An Out-of-Home Care Health Screen
  • A Comprehensive Initial Health Assessment
  • Behavioral and Mental Health Screening
  • All Medicaid-covered benefits
  • Dental and Vision care
