Child Care Certification Policy Manual

Family and In-Home Child Care (DCF-F-2665-E) is available on the Certification SharePoint site.
Effective 1/1/20 certification workers will use a system-generated compliance statement because they will be required to enter rules met.
See Section 12 for guidance regarding use of the Compliance Statement form.
Noncompliance Statement and Correction Plan (often referred to as “294”) The form is available on the Certification SharePoint site, however, use of the WISCCRS generated form is preferred. See sections 10 and 11 for guidance regarding use of the Noncompliance Statement and Correction Plan form.
Site Visit Comments/Recommendations (optional)
See Section 20 for guidance regarding use of the Site Visit Comments/Recommendations form.
Exit Interview (optional). See Section 19 for guidance regarding use of the Exit Interview form. This form is available on the Certification SharePoint site and document sales.
Records Removal Receipt (optional) The form is available on the Certification SharePoint site. See Section 14 for guidance regarding use of the Records Removal Receipt.
Staff and Child Identification Key (optional) The form is available on the Certification SharePoint site. See Section 10.4 for guidance regarding use of the Staff and Child Identification Key.

These optional templates may be modified by certification workers to provide instruction to child care operators on completing a Plan of Correction and are available on the Certification SharePoint site.
