Child Care Certification Policy Manual
This form may be used by certification workers to communicate comments or recommendations resulting from a site visit, or other communication to the certified operator, such as enumerating standards that need to be met before regulation may be granted. The information the certification worker documents on the form is not to be construed as citations of violations. Any violations resulting from a site visit shall be communicated and documented on a Noncompliance Statement (exception initial and relocation visits) and on the Standards and Checklist form.
For example, if a certified operator moves to a new home but during the relocation inspection, the certification worker discovers the home is not equipped with carbon monoxide detectors on every level of the home, the certification worker might choose to convey this information on the Site Visit Comments / Recommendations form, in addition to the documentation on the Standards and Checklist.
The form may also be used to document and communicate information to the operator as a result of an initial visit or technical assistance visit.
The Site Visit Comments / Recommendations form may be used in addition to a Noncompliance or Compliance Statement but may not be used in lieu of either of these forms. All monitoring visits (with exception of initial and relocation) shall result in issuance of either a Compliance or Noncompliance Statement.
