9.4 - Confidential Information

Names, dates of birth or any other information that might be used to identify an individual should not be entered on the Noncompliance Statement. Confidential information also includes mental health, alcohol and other drug abuse, child protective services, juvenile delinquency information or children’s names (including the operator’s / provider’s children).

 

If a violation is about missing information for a child or staff person’s file or if the violation is about the actions of one or more persons, the identifying information can be documented using a confidential key. The confidential key could be the standards and checklist or the department’s Staff and Child Identification Key form. The key is then shared with the operator but is not to be posted on the department’s Regulated Child Care and YoungStar Public Search website.
