4. Reporting Monitoring Visit Results

Once all rules have been monitored and determined to be met, unmet or not applicable, these results are documented by the licensing specialist in the mobile app and this information is automatically entered into the DCF-294A or DCF-785C. (Less often, the DCF-294A may also be used to enumerate and document one or more violations discovered as a result of a self-report by the public school program.) Only violations for administrative rules contained in the Annual Health and Safety Rule Tag are cited on these forms.
