4.2 Changes to the DCF-294A

It is possible that the licensing specialist is uncertain about whether or not something observed at the public school program should be cited and will decide this later, after further research and consultation with other licensing staff or supervisors. In this case, the licensing specialist should indicate to staff that there is a possibility that there could be one or more additional violations that may be cited, and that s/he will call or email the program if additional violations are included. Program staff can be encouraged to contact the licensing specialist with questions.

If the violations discussed with the public school program at the exit interview are subsequently changed, the licensing specialist should make every effort to contact the program by telephone, email, or regular mail and inform the provider of any changes to the DCF-294A in advance of receiving the final revised version.
