When no violations are cited, the licensing specialist completes and issues the DCF-785C.
When violations are cited, the DCF-294A is completed, providing a clear statement of how the program is out of compliance with each identified rule. The licensing specialist documents his/her observation by indicating ‘unmet’ and describing the violation in the External Comments field in the mobile app. This description entered in this field automatically fills in the column labeled Rule/Statute Number - Noncompliance Statement on the DCF-294A.
When the licensing specialist has completed the DCF-294A or DCF-785C, s/he signs and dates the form and enters his/her telephone number. The form is mailed, emailed, or hand delivered to the program as soon as possible, but no later than 10 business days after the site visit (or receipt of a self-report). (This time line is not applicable if an incident/accident investigation is underway and/or further information must be obtained following the site visit.)
If possible, the visit results should be shared with the person designated in charge of the program on the day of the visit. (See section 6.) The licensing specialist offers to conduct an exit interview before leaving the public school program to inform the program staff of the visit results, including probable violations that will be cited.
