Child Care Certification Policy Manual
Certification agencies shall provide application packets / materials to prospective child care applicants and include the following:
Application for Family and In-Home Child Care Certification
W9 Form (no longer required effective 1/1/21 if using Application for Family and In-home Child Care Certification with revision date 12/20 or later)
Regulatory Agency Approval Form (if applicable)
YoungStar Contract English / Spanish or Wisconsin Shares Contract

Certification Forms and Publications
The most up-to-date forms are available on the certification forms web page, however, specific forms can be ordered through the Document Sales and Distribution website at no charge. Click on Department of Children and Families link in left hand margin for list of DCF forms or use the product search in top margin. If you are a first-time customer using the document sales link, you will need to click on the new customer link in top right corner. It is important to keep in mind, however, that pre-printed forms from Document Sales are not always updated timely like they are on the certification forms web page.
Note: Before making your own copies of any department-provided forms check the certification forms website for any updates to forms, as the certification website will typically have updated forms before document sales will have a printed supply available.
Individuals wishing to apply for certification shall submit required application forms and information (outlined below) to the certification agency with jurisdiction. See Certification Policy Module 1, Section 8 ‘Jurisdiction’ for more information.
It is recommended that all documents submitted by the applicant / operator are date stamped with the received date. The date stamp is useful when entering WISCCRS data and / or determining compliance with certification standards.
