Child Care Certification Policy Manual

2.2 - Initial Application - Certified Family / In-Home Child Care

Certification agencies shall provide prospective applicants with information about the application process. It is preferred the applicant submit an online application for certification using the Child Care Provider Portal. Application (paper or online) includes the following:

Application for Family and In-Home Child Care Certification
Tuberculosis Risk Assessment/Screening
Standards and Checklist
Background Check Request (BCR)
- Spanish BCR
- Hmong BCR
Regulatory Agency Approval Form (if applicable)
YoungStar Contract English / Spanish or Wisconsin Shares Contract

Individuals wishing to apply for certification shall submit the Expression of Interest and online (or paper) application forms to the certification agency with jurisdiction. The applicants must select the child care program location county/tribe when submitting the EOI. This will ensure the EOI Dashboard Task is sent to the correct county/tribe (certifying agency). See Certification Policy Module 1, Section 8 ‘Jurisdiction’ for more information.

It is recommended that any paper documents submitted by the applicant / operator are date stamped with the received date. The date stamp is useful when entering WISCCRS data and / or determining compliance with certification standards.
