2.2.12.1 - YoungStar Contract - for Family Child Care Operators or Wisconsin Shares Contract - for In-Home Family Certified Child Care Operators

YoungStar Contract is used for Family Child Care Providers

Wisconsin Shares Contract is used for In-Home Family Child Care Providers

• Forms are available in English, Spanish and Hmong and are linked to the certification forms page.

• Certified operators that receive Wisconsin Shares Child Care Subsidy are required to participate in YoungStar and must submit a YoungStar or Wisconsin Shares Contract.

• The contract shall be submitted to the local YoungStar Office, however, if the applicant / operator mistakenly submits the form to the certification agency, the agency shall date-stamp it and forward on to the local YoungStar Office.
