Child Care Licensing Procedure Manual
The CFS-785 is used in conjunction with a monitoring visit to a licensed facility to recognize compliance when, during a monitoring visit, the licensing specialist observes no violations for the selected administrative rules under review at that time. There are four versions of the CFS-785:

These forms are WISCCRS-generated. There is also a Word-fillable version of the CFS-785 on the BECR Forms & Publications web page which may be completed and issued to a camp or center.
The CFS-785 identifies child care administrative rule sections. To complete the CFS-785, the licensing specialist indicates that no violations were observed and cited in a particular rule section by placing a check mark beside each section that was reviewed during the monitoring visit. If the licensing specialist didn’t monitor the entire section, s/he may enter the specific subsections, paragraphs and subdivisions of the rules that were monitored.
Issuing a CFS-785 does not mean that:
There are no rule violations existing in the center or camp;
Violations would not have been cited had the licensing specialist monitored a different section of the administrative rules during the site visit;
There were no violations of the same or different rules cited in a previous monitoring visit;
There will not be a violation of the same or different rules at a subsequent visit.
