Child Care Licensing Procedure Manual
The CFS-785 may be completed by the licensing specialist at the facility and presented during the exit interview. The licensing specialist enters the facility information, signs and dates the form and has the licensee do the same. A copy of the form is provided to the licensee.
If the licensing specialist prefers to complete and issue the electronic CFS-785 form after the visit, it must be done as soon as possible and no later than 10 business days from the date of the site visit. A copy is printed and mailed, emailed, or hand delivered to the facility. In this case, the licensee’s signature is not required but is considered good practice.
Compliance information is promptly submitted to WISCCRS as soon as possible after the monitoring visit. Compliance information is displayed on the Regulated Child Care and YoungStar Public Search website one day after the issue date is entered in WISCCRS. Compliance information remains on the public website for three years.
