Informal Assessment Summary

The “Informal Assessment Summary” page provides an overview of the informal assessment and allows you to access history for each page of the informal assessment. If you have read-only access to WWP, the “Informal Assessment Summary” is the way you will view informal assessment data.

Accessing the “Informal Assessment Summary”

The “Informal Assessment Summary” page can be accessed via the “Case Management” menu once a participant has been selected:

Collapsing and Expanding Sections

The “Informal Assessment Summary” contains a lot of information. To make reading through the page easier, you can collapse sections that you don’t need to look at right now. You can do this by clicking the arrow icons shown below:

Once a section is collapsed, it will look like this:

You can expand the section by clicking on the arrow icon again.

Viewing History for a Section

You can view history for a section by clicking on the history icon, as shown below:

Clicking on the history icon will allow you to see when the page was last edited and who it was last edited by:

You can also navigate to a previous version of the page by clicking the arrow icon next to the date and time:

If you are not on the most recent record, you can click the right-hand arrow to move to a more recent record.

You can use the dropdown menu to see a list of all edits to the page, as shown below:

Select any item on the list to view that record.

Help for Individuals with Read-only Access

If you have read-only access to WWP, the informal assessment summary is your way to view data for the informal assessment. Several helpful tips to keep in mind:

  1. The informal assessment summary uses shortened wording for some questions. To see the full wording of a question, mouse over the text, as shown below:

  2. If a question is displayed with a grey background, that means that it was optional, but not completed. An example is shown below:

  3. If a question was not displayed to the case manager, it will not be displayed on the informal assessment summary.
