If the licensing specialist issues a new CFS-294 to the facility before the expected correction dates for the violations on the previously issued CFS-294, and the corrections have not yet occurred, the licensing specialist includes the uncorrected violations from the previously issued CFS-294 by attaching the previously issued CFS-294 to the new CFS-294. Previously cited violations cannot be added to the CFS-294 because the re-entry of the violations into WISCCRS will result in an inaccurate designation of ‘repeat’. (When violations are entered in WISCCRS, the system automatically identifies violations that have been cited within the previous 24-month period and an informational alert will display indicating repeat violation.) The licensee must post both the previously issued CFS-294 and the new CFS-294.
The licensing specialist should monitor verification of both sets of violations at the next site visit. As violations are verified as corrected by the licensing specialist, the licensing specialist marks each rule as met in the mobile app. If the verification occurs on-site, the licensing specialist indicates the verification date of each corrected violation on the licensee’s copy of the CFS-294 and initials the entry.
If the expected completion date passes without correction of the violations, the licensing specialist should consider it to be a repeat violation.
