The licensing specialist can verify that all violations included on the CFS-294 are corrected during the next site visit to the facility or through review of mailed, emailed, or hand delivered documents showing correction of all violations.
When doing a verification on-site visit, if all violations have been verified as corrected, the licensing specialist marks the previously unmet violations as met in the mobile app. S/he should also complete the verification column on the licensee’s posted copy by entering the verification date (the date of the site visit or the date of receipt of written documentation) in the Verification Date column on the form. S/he initials the entry.
Although all violations may have been corrected, the licensing specialist does not issue a CFS-785. The correction of the violations cited on the previously issued CFS-294 is documented on the form and the licensing specialist should instruct the licensee to continue to post the CFS-294 with all corrections noted as corrected, until the next CFS-294 or CFS-785 is issued. When new violations are observed during the site visit, a new CFS-294 is issued.
The licensing specialist may issue a CFS-785 while verifying correction of previously cited violations if the licensing specialist conducts additional monitoring, beyond verification of corrections from the previous CFS-294, and no new violations are cited during the additional monitoring.
When staff records are reviewed at a central location for multiple facilities that cross regional boundaries and violations are cited, the licensing specialist assigned to the facility that was issued the CFS-294 is responsible for verifying correction of the violation. Correction may be verified through submittal of documentation either via email or through regular mail.
