9.2.1 W-2 Supportive Service Plan
The goal of a Supportive Service Plan is to ensure participants and their families have the resources they need to address family and work-related needs.
The Supportive Service Plan must include:
- Information necessary to connect applicants and participants with supportive services available both within the agency and in the community.
- Proactive strategies for addressing family and work needs before these situations result in a family crisis and/or loss of employment.
