The goal of a Supportive Service Plan is to ensure participants and their families have the resources they need to address family and work-related needs.
The Supportive Service Plan must include:
1. Information necessary to connect applicants and participants with supportive services available both within the agency and in the community.
2. Proactive strategies for addressing family and work needs before these situations result in a family crisis and/or loss of employment.
