Under W-2 , assessment is the process of gathering information to develop an Employability Plan (EP) customized for the participant that will result in either a:
· Successful employment outcome to start the individual on a career path; or, if appropriate,
· Path to eligibility for SSI , SSDI , or both.
Certain types of assessments are required for all W-2 applicants and participants, including:
· Informal Assessment (see 5.2); and
· Educational Needs Assessment (see 5.3).
The information gathered through these required assessments help the FEP in identifying whether additional Formal Assessments (see 5.5) are needed to develop the participant’s EP .
The assessment process is conducted in collaboration with the applicant or participant. The FEP must:
· Discuss the results of each assessment with the participant; and
· Work together with the participant when deciding which W-2 placement and activities are assigned as part of the EP.
