Child Care Licensing Procedure Manual
All investigative findings must be written up and closed out within 20 working days from completion of the investigation, unless the regional licensing manager/licensing supervisor grants an extension.
The licensing specialist documents the complaint investigation findings on the following forms:
Child Care Complaint/Incident Findings form (CFS-321-B). The purpose of this form is to document the complaint investigation and to support subsequent actions taken. Unless requested through an open record request, this is not released to the licensee and complainant. (See section 12.);
If requested by the complainant or licensee, Complaint Investigation Summary Letter. The purpose of this letter is to provide a written summary of the investigation findings and recommendations. It is not issued unless the complainant or licensee specifically requests a written complaint results summary;
If warranted, Noncompliance Statement and Correction Plan (CFS-294). For more information, see the chapter titled Noncompliance Statement and Correction Plan (CFS-294) and Compliance Statement (CFS-785) in the Monitoring section of the manual.
Information on the completed CFS-321-B and, if applicable, the CFS-294, are entered into WISCCRS, uploaded to the center file and shared with the licensing manager/licensing supervisor.
