Child Care Licensing Procedure Manual
Wis. Admin. Codes §§ DCF 250.04(3)(a), 250.04(3)(am), 250.04(3)(ar), 251.04(3)(a), 251.04(3)(am), 251.04(3)(ar), 252.41(2)(a), 252.41(2)(am) and 252.41(2)(ar) require family child care, group child care and day camp licensees (or a person acting on behalf of the licensee) to self-report to BECR any incident or accident that results in professional medical evaluation, death, or injury caused by an animal that occurs while the child is in the care of the center, within 24 hours of the licensee becoming aware of the medical evaluation, or death, or animal injury.
This means that reporting is required for:
An incident or accident resulting in professional medical evaluation that occurred while the child was in care at the center and the medical evaluation was immediately sought by the licensee or center staff, or;
An incident or accident resulting in professional medical evaluation that occurred while the child was in care at the center and the parent or guardian sought medical evaluation after the child left the center. Once the parent or guardian informs the licensee that medical evaluation was sought, the licensee must inform BECR within 24 hours, or;
The initial event resulting in the death of a child in care, whether due to an accident or to natural causes, occurred while the child was in the care of the center, regardless of when medical evaluation was sought or by whom. For example, a child’s asthma attack that first occurred at the child care center and later resulted in the child’s death at a medical facility must be reported to BECR. However, a child’s asthma attack that occurred over the weekend, when the child was not in the care of the center and then resulted in the child’s death, is not a required reportable death, or
An injury caused by an animal to a child in care.
