Wisconsin Shares Suspended/Terminated Providers

Below is a list of child care providers recently suspended from the Wisconsin Shares program by the Department of Children and Families (DCF), because DCF reasonably suspects program violations, as allowed by new Wisconsin law 49.155 (7) (b) 4 or 49.155(7m).

These cases continue to be investigated and where applicable, overpayment collections pursued. The content below will be updated as additional information is gathered through the ongoing investigations. If the investigation reveals information that removes DCF's reasonable suspicion of program violations, child care providers will be removed from this list and their suspension from Wisconsin Shares will be lifted.

The providers here are suspended from the Wisconsin Shares program and therefore will not be receiving Shares payments, however it does not mean their licenses to provide care have necessarily been revoked. The investigations evaluate providers case-by-case and some providers may have revoked licenses or certifications or have chosen to voluntarily close.

All suspended providers have the right to appeal their suspensions to the Division of Hearings and Appeals within 30 days of the effective date of the suspension.

2018 Suspended Child Care Providers

 

7000588977 Jellybeans Learning and Playland/Jean Hurt-Taylor/Milwaukee County

  • Suspension Date: 08/17/2018
  • OVERBILL: Billed for more hours than children actually attended
  • TIME CHANGES: Made changes to the initially recorded arrival and departure times on your attendance records
  • MONITORING VISIT DISCREPANCIES: The provider billed and/or received payment for children not observed in attendance during monitoring visits by the Department or other agency employees.
  • NOT OBSERVED: Billed for children who were not in attendance as they were not observed in care
  • CLOSED: Billed for children when your facility was closed
  • CARING FOR CHILDREN OUTSIDE OF REGULATED HOURS: The provider cared for children while operating outside of their regulated hours
  • UNQUALIFIED PROVIDER: The staff provider care for the children were not qualified

  • Suspension Date: 08/17/2018
  • NO ACCESS TO FACILITY: DCF 201.04(6)(e) authorizes the department or child care administrative agency to make on-site inspections to monitor provision of authorized services.
    • The Department has been unable to verify the actual care of children at your facility.  The Department attempted to visit your facility on 06/06/2017,  07/10/2017, 10/03/2017, 10/10/2017, 10/18/2017, 01/18/2018, 01/23/2018, 02/05/2018, 02/19/2018, and 03/07/2018 and was unsuccessful in gaining access to the facility. The unsuccessful attempt(s) to verify the presence of children attending your facility raises a strong suspicion that you were billing the Wisconsin Shares program for children who did not actually attend your child care facility.
    • The Department reviewed your billing records and confirmed you billed for children as being in attendance on 10/03/2017, 10/18/2017, and 03/07/2018 even though the Department was unable to observe any children in attendance.
  • SURVEILLANCE DISCREPENCIES: Surveillance shows the provider inaccurately billed for children in attendance

  • Suspension Date: 05/30/2018
  • ATTENDANCE RECORDS NOT AVAILABLE: § 49.155 (6m) With respect to attendance records, a child care provider shall do all of the following:
    • (a) Maintain a written record of the daily hours of attendance of each child for whom the provider is providing care under this section, including the actual arrival and departure times for each child.
    • (b) Retain the written daily attendance records under par. (a) for each child for at least 3 years after the child’s last day of attendance, regardless of whether the child care provider is still receiving or eligible to receive payments under this section.

The Department attempted to visit your facility on 03/01/2017, 04/04/2017, 10/18/2017, and 10/31/2017 and was unsuccessful in gaining access to the facility. The unsuccessful attempt(s) to verify the presence of children attending your facility raises a strong suspicion that you were billing the Wisconsin Shares program for children who did not actually attend your child care facility.
    • The Department reviewed your billing records and confirmed you billed for children as being in attendance on 04/04/2017, 10/18/2017, and 10/31/2017 even though the Department was unable to observe any children in attendance.

  • DUPLICATE RECORDS: Billed for children while maintaining multiple sets of attendance records with contradicting arrival and departure times of children.

    • The Department reviewed your billing records and confirmed you billed for children as being in attendance on 7/13/2017 and 8/8/2017 even though the Department was unable to observe any children in attendance.
