12.2.11 Fact Finder File

The Fact Finder must create and maintain a complete Fact Finding file. The file must provide a complete and thorough record of the Review and must include:

  1. Request for Wisconsin Works (W-2) Fact Finding Review (10783) or its equivalent;
  2. Fact Finding Review Appointment Notice (10782);
  3. Fact Finding Review Voluntary Withdrawal (11155), if relevant;
  4. Fact Finding Review Summary and Decision (10784), or its equivalent;
  5. Documents Submitted at Fact Finding (370);
  6. Any recordings of the Fact Finding Review; and
  7. Any additional information or evidence presented by the W-2 agency, the PetitionerAn individual requesting a Fact Finding or Departmental review., or the petitioner’s representative.

All Fact Finding Review documentation, including any additional information or evidence presented by the W-2 agency, the petitioner, or the petitioner’s representative, must be scanned into the petitioner’s Electronic Case Record.

The petitioner, or petitioner’s representative, has the right to obtain copies of any part of the Fact Finding file, including the recording. The agency is responsible for providing any requested documentation. If the cost of providing the requested documentation is more than $30, repayment may be sought. (See 12.2.6)
