4.1.4  Frequency of Verification

The following items must only be verified once per lifetime:

·        Identity;

·        Social Security Number;

·        Birth date; and

·        Citizenship.

To ensure these items are only verified once, the FEP must scan copies of the documents used to verify these items into ECF in the appropriate folder based on the document type. See the ECF User Guide Document Lists and Codes for more information on document types.

Applicants do not need to resubmit verification for identity, SSN, birth date, or citizenship if verification of these items is stored in ECF. If an applicant has one of these items stored in ECF from a previous case, The FEP must:

1.             View the verification documents stored in ECF from the previous case; and

2.             Enter case comments documenting that these items were viewed in ECF and previously known to CWW.

Other eligibility criteria should be reviewed at every eligibility review and when new information is reported or received through a data exchange. (See 4.1.5)
