The CMD placement is for individuals who have reached their 24-month placement time limit or 48-month state lifetime limit and are no longer eligible for a paid placement. When a participant is denied or declines an extension, the FEP must offer the CMD placement.
To be eligible for the CMD placement, the participant must meet W-2 financial and nonfinancial eligibility criteria (see 2.2.1), with the exception of exceeding the 24-month placement time limit or 48-month state lifetime limit.
The goal of the CMD placement is to:
1. Match the individual to employment;
2. Connect the individual and their family to resources and services; and
3. Reassess, on a monthly basis, whether the individual is eligible for an extension.
EXAMPLE: Karl has been a CSJ participant for 18 months and assigned the following activities:
· Life Skills;
· Job Readiness/Motivation;
· Mental Health Counseling for his daughter Kassandra;
· Work Experience; and
· Employment Search.
After missing multiple activities without good cause, Karl’s FEP referred him for a formal assessment with a mental health provider. However, Karl did not follow through with the referral. As Karl approached his 24th month, Karl’s FEP attempted to reach him by phone and mail a number of times to discuss an extension, but Karl never responded. In Karl’s 22nd month, Karl contacted his worker and told them that he was having a hard time. Karl’s FEP recognized that Karl may be suffering from depression and that without additional support to help Karl participate, that they would have to deny eligibility for another time limit extension.
Karl’s FEP gave him a three-month extension and, in addition to Work Experience and Employment Search, scheduled a new referral for a formal assessment with a mental health provider that provides virtual appointments so Karl would not have to leave his house.
Karl participated sporadically in the assigned activities but never followed through on the second formal assessment referral. The FEP denied eligibility for a subsequent time limit extension due to Karl’s nonparticipation and placed Karl in a CMD to try and assist Karl in finding employment.
Services provided to a CMD participant will depend largely on the:
· Reason the extension was denied or declined by the participant; and
· Placement the participant was in when they declined or were denied an extension.
Participants who are denied an extension due to nonparticipation may be assigned activities similar to activities assigned while in the subsidized employment position.
FEPs must meet with CMD participants:
· Weekly, to help the participant find employment and connect them with needed resources; and
· Every 30 days to review the CMD placement. (See 1.4.2.4)
During the 30-day review, the FEP must reassess the case. If the participant has consistently participated, the FEP must reassess for potential barriers to employment and determine whether an extension is appropriate.
A participant in the CMD placement who finds unsubsidized employment may be placed in the CMU placement. FEPs should document this in PIN comments.
