When obtaining formal assessment information for a participant with a medical or mental health condition, the W-2 agency should first consult with any providers treating the participant for the condition.
If documentation provided does not contain information needed to determine an appropriate placement or necessary accommodations for the participant, the W-2 agency may refer the participant to another service provider for an independent assessment. (See 5.5.3)
If assessment information is gathered from multiple sources, and the information about the medical condition or the recommended services or accommodations needed conflicts, the agency should use the information from the provider who has provided an independent assessment.
If a participant has multiple service providers for different medical conditions and they provide different opinions about the accommodations needed, the FEP must assign activities and provide needed accommodations based on both assessments.
