4. Records Disposal

When facility files are purged according to RDA standards, any item from the file with personally identifiable information must be shredded. Personally identifiable information includes, but is not limited to, social security numbers, federal identification numbers, children’s names, children’s addresses, children’s phone numbers, etc. Paper copies of fingerprint results contained in a paper file of a closed facility must be purged and shredded before the file is sent to the State Records Center (or, if stored on-site, the regional licensing office storage area).
