7.2 When the Public School Program Does Not Self-Report a Reportable Incident

When a licensing specialist becomes aware of an event that the public school program failed to report, such as a discovery during a complaint investigation of a reportable injury that was not reported to BECR, the licensing specialist instructs the licensee to submit a required written self-report of the incident and issues a violation on the DCF-294A for failure to report. The licensing specialist conducts a follow-up investigation just as if it had been self-reported, and those results are recorded in the Complaint/Incident Reporting Module.
