At the end of the initial on-site visit, the licensing specialist discusses his/her observations with the program director (or person in charge on the day of the visit) and provides a copy of the completed DCF-5154 checklist, identifying the results of the walk-through. If it is not possible to make a copy of the checklist on site, it is sent electronically or via US mail to the program within 10 business days of the visit. Either way, the licensing specialist must provide the program with the document to identify those rules that will need to be met at future monitoring visits in order to be in compliance with health and safety standards.
The copy of DCF-5154 should be retained and reviewed by the licensing specialist when making the first annual monitoring visit to the public school program following the initial on-site visit.
