Wis. Stat. § 48.66(3) requires the Department to prescribe the information to be reported by persons licensed by it. Wis. Admin. Code § DCF 251.04(3)(d) requires that a group child care center report to the BECR regional office a change of any program service, at least five days prior to the change. This reporting requirement applies to voluntary temporary closures.
If the licensee initially reports the change to BECR verbally, s/he is required to submit a written report via Fax, e-mail, or letter. Although the administrative rule does not specify the format the licensee must use to report a change in service, the format preferred by BECR for reporting and documenting this notification is the Notice of Voluntary Temporary Closure of Child Care Center, available on the BECR Intranet. The Notice requests information from the licensee that will define the terms of the closure and is signed and dated by the licensee and the licensing specialist.
