3.5 Initiating a CFS-294 for Failure to Submit Required Continuation Materials

A CFS-294 is issued to licensees who fail to submit required continuation materials at least 30 days prior to the continuation expiration date. A regional program associate is responsible for preparing and mailing or emailing to the licensee the CFS-294 indicating the rule violation along with a reminder letter that informs the licensee that BECR will verify correction of the violation when materials and fees are submitted.

The CFS-294 that is sent to the licensee has the Correction Plan column pre-filled with the statement, “Submittal of all continuation materials/fees,” and the Expected Completion Date column pre-filled with the continuation expiration date. The program associate uploads a copy to WISCCRS and makes sure that the assigned licensing specialist is aware of the violation. S/he enters the violation data into WISCCRS with a Visit Reason of “DESK REVIEW."

The licensee must post the CFS-294 beside the license.

When all continuation materials and fees have been received, the noncompliance is considered verified as corrected. A licensing staff person (either the program associate or the licensing specialist) enters the date the required application materials were received in the Verification Date column of the CFS-294 and initials the entry. A copy of the verified CFS-294 should be provided to the licensee.
