When the licensing specialist receives a self-report and confirms that one or more violations have occurred, the licensing specialist documents the violation on a CFS-294. The CFS-294 describes the violation and includes the notation, “Self-Reported on (date)” in the Rule/Statute Number - Noncompliance Statement column. The CFS-294 is delivered to the licensee as soon as possible, but no later than 10 business days from the date the licensing specialist determined a violation occurred. The licensee is instructed to enter what will be done to correct the violation in the Correction Plan column of the form and the date this will be accomplished in the Expected Completion Date column. The licensing specialist monitors the return of the CFS-294 and assesses the acceptability of the correction plan. The form is then posted at the facility near the license and on the public search website.
Sometimes the licensee reports that corrective action has already been taken to address the violations identified in the self-report. In this case, the same procedure described above for issuing the CFS-294 is followed except that the licensee is instructed to enter precisely what has already been done to correct the violation in the Correction Plan column of the form. The licensee indicates “Corrected” and the date it was corrected in the Expected Completion Date column. The licensing specialist monitors the return of the CFS-294. The form is posted at the facility near the license and on the public search website.
If a violation from a previously issued CFS-294 has not yet been verified as corrected, the licensee should post the newly issued CFS-294 along with the previously issued CFS-294. The licensing specialist will verify correction of all cited violations at the next monitoring visit.
If the licensing specialist determines that the self-reported event is not a violation of licensing rules, the licensing specialist should inform the licensee of this, either by email, telephone, or in person. A CFS-785 is not issued in this case.
