A complaint investigation may involve multiple visits to the center or camp and violations may be observed at each of the visits. All violations directly related to a complaint should be cited on the same CFS-294. Any violations that may be incidentally observed during the complaint investigation visits, including violations unrelated to the complaint allegations under investigation, should also be cited on this same CFS-294.
The CFS-294 is issued at the end of the complaint investigation. The CFS-294 date is the date of the last visit of the complaint investigation. When violations are observed on different dates, the date that each violation is cited should be identified in the description of the violation so that those dates will be noted in the column labeled Rule/Statute Number - Noncompliance Statement.
If the licensing specialist observes a violation that requires immediate correction during the complaint investigation, such as a dangerous playground hazard, a CFS-294 citing that particular violation may be issued immediately following the visit, if the licensing manager/supervisor agrees.
The time frame for delivering a CFS-294 related to a complaint investigation is 20 business days after the conclusion of the complaint investigation. For more information regarding complaint investigations, see the Complaint Investigations – Licensed Facilities section in the procedure manual.
