12.1 Recording the CFS-294 and CFS-785 in WISCCRS

The facility file is the collection of all information and documentation maintained by BECR pertaining to a licensed child care center or day camp. This includes the facility information entered into WISCCRS and the documents that are collected, reviewed and retained for each licensed facility. The facility file consists of all records, whether in the electronic format, uploaded and stored in WISCCRS, or in hard copies retained in a paper facility file. Licensing staff must insure that all documented rule violations included on the CFS-294 as well as the compliance information included on a CFS-785 are promptly recorded in the WISCCRS facility file.

The procedure chapter titled Facility Files identifies the WISCCRS location for all facility file documents contained in a current, active licensed child care or day camp facility file. The CFS-294 and CFS-785 are uploaded into the Site Visits section of WISCCRS.

All CFS-294 and CFS-785 information is displayed on the Regulated Child Care and YoungStar Public Search website the day after an issue date is entered into WISCCRS. After the licensing specialist receives and reviews the licensee’s plan of correction, the licensee’s plan is also scanned into WISCCRS and displayed on the licensee’s page on the public search website. Violations and compliance information remain on the public search website for three years. Once three years has passed since a violation was cited, the violation automatically disappears from the website.
