11.2 Citing Repeat Violations

The licensing specialist should record the uncorrected violation on a new CFS-294 along with any newly identified violations resulting from a site visit. When the violation is a repeat violation, the CFS-294 form will display the date it was previously cited. The CFS-294 process repeats again, and the violation that was previously cited but not corrected will be monitored at the next site visit. Similarly, if a violation has been verified as having been corrected but is subsequently observed by the licensing specialist, she/he should cite it as a violation on a new CFS-294 and “Repeat Violation” will be identified with the previous citation date in the Rule/Statute Number - Noncompliance Statement column.
