Temporary Closure
Under Location Details, include documentation that a center is temporarily closed, begin and end dates for the closure and the reason for the closure;
Example: “The center is temporarily closed due to REASON from BEGIN DATE to END DATE.”
Licensed Hours and Operating Hours
Under Location Details, include documentation of licensed hours and operating hours when current operating hours differ from licensed hours.
Example: “Licensed hours are BEGIN TIME to END TIME. As of DATE, licensee is operating from BEGIN TIME to END TIME.”
Change of Schedule
Under Location Details, document a change in the days of week that program is open.
Example: “As of DATE, program will be open on Saturdays from 8:00 AM – 1:00 PM for the summer, in addition to weekday schedule.”
License Amendments
Under Location Details, document license amendments.
Example: “As of DATE, Ages Served has been amended To 6 Wks - 12 Yrs, 11 Mos.;
Designation of Authority
Under Location Details, include notation of the name of the person currently designated to sign on behalf of the licensee.
Example: “NAME is authorized by NAME OF LICENSEE to sign licensing documents on behalf of the licensee.”
Caregiver Background Check
Under Location Details, include identification of the member of the board of directors who is subject to the caregiver background check, if it isn’t the board president;
Example: “Board President John Doe authorizes Board Vice President Jane Doe to be the Caregiver Background Check designee.”
Relocation
Under Location Details, include notification of a relocation.
Example: “Provider has temporarily relocated to 123 Maple Ave., Any Town, for 2 weeks (BEGIN DATE – END DATE) until water damage to classroom is repaired.”
Facility Number
Under Location Details, include a notation of the Facility Number previously assigned to the provider.
Example: “This facility was previously licensed under Facility Number XXX”
Capacity Change
Under Location Details, document a capacity change and the date of the change;
Example: “As of DATE, capacity increased from 100 to 150.”
Next Visit Notes
Under Location Details, the licensing specialist may make notes regarding the next visit, such as a notation regarding the entrance, pets on the premises, or other observations that would be instructive to staff persons visiting the facility.
Example: “Use side door next to garage, not front door.”
