Contact with the Provider or Provider’s Staff
Include the date (and time, if deemed relevant) of a contact with the provider or provider’s staff, whether via telephone, email, personal visit to the office, etc. The conversation with a provider or provider’s staff is not described in detail but the purpose or reason for the contact is noted.
Example: “On DATE, NAME called to request information regarding requirements for attendance recording. Explained rule requirements.”
Example: “On DATE, NAME emailed to request clarification of a violation cited on CFS-294 on DATE. Responded by email with explanation of violation.”
Example: “On DATE, NAME made a visit to the regional office to inform BECR that provider will no longer provide transportation.”
Attempted Visit
Include documentation of descriptive information regarding an attempted visit or other details regarding an incomplete visit (e.g., no one home, children were not present).
Example: “No one answered repeated attempt to gain entry, including knocking on front door and ringing doorbell. The house appeared dark.”
Exit Interview
Include documentation that an exit interview was conducted, including when the exit interview was conducted, who was present and any outstanding comments or questions that were noted at that time.
Example: “An exit interview was conducted on DATE at TIME with NAMES OF ALL PERSONS PRESENT. NAME requested clarification of NOTE ANY REMARKABLE QUESTIONS OR COMMENTS.”
Decision to Waive Requirement for Order
Include documentation of the reason for the decision to waive the procedural requirement to issue an order when the licensing specialist is citing at least four repeat, serious violations during a single visit.
Example: “On DATE, the licensing manager approved the decision to waive the requirement to issue an order due to four, repeat serious violations because the licensee has made significant progress completing the required repairs resulting in the repeat violations (previously cited on DATE). All violations are expected to be verified corrected within one month, on DATE.”
