6. Assigning Facilities to a Monitoring Plan

The monitoring plan establishes the minimum number of monitoring visits the licensing specialist must make to the center in a 12-month period. Although the monitoring plan defines the minimum number of visits, licensing specialists may make additional visits to the licensed center as time permits and to assure compliance with licensing rules.

The licensing specialist must review all administrative rules identified on the licensing checklist for each licensed center within a two-year period. In addition, the licensing specialist must annually review all administrative rules identified in the Annual Health and Safety Rule Tag.

Licensing specialists are responsible for assigning each licensed center to a monitoring plan according to the center type, size, probation status and previous compliance with administrative rules. The licensing specialist assigns the center to one of the monitoring plans listed below. Monitoring plans are updated by the licensing specialist when an enforcement action is issued, an enforcement action is verified and closed, the facility is transitioning from a probationary to regular license, or there are other reasons to change the plan as a result of the monitoring visit, such issuance of numerous violations.

The guidelines for assigning a center to a monitoring plan are as follows:

6.1 Minimum Monitoring Plan

6.2 Moderate Monitoring Plan

6.3 Maximum Monitoring Plan

6.4 Probationary License Plan

6.5 Unlicensed Provider/Center Plan
