A Family Child Care/Foster Care Stipulation is identified on the license certificate. On the license certificate itself, under the Conditions heading, the stipulation is identified with the statement, “Family Child Care/Foster Care Stipulation dated mm/dd/yyyy”. In order to have the identification of the stipulation printed on the license certificate, the value “Stipulation” must be picked from the drop down options under Condition Type in WISCCRS and the 'Print on Certificate' box must be checked. The statement “Family Child Care/Foster Care Stipulation dated mm/dd/yyyy” must be entered into the comments field.
