The original notice of forfeiture assessment is mailed via regular US mail and certified mail or hand-delivered to the licensee. If hand-delivered, the licensing specialist must obtain the appropriate signature on the Delivery of Document Receipt.
The signed notice of forfeiture assessment and the certified mail delivery date receipt (or the BECR Receipt of Delivery form) must be uploaded in WISCCRS. A copy of the forfeiture notice is provided to the DCF Office of Legal Counsel and to the BECR Central Office Manager.
